Guidelines for completion of CE Documentation:

After the planning process is complete, the next step is to complete and submit the documentation:

General Guidelines:

  • One contact hour equals 60 minutes
  • Documentation must be received at least four weeks prior to the date of the program.
  • Contact hours expire two years after the date the program is first offered
  • For programs that are co-sponsored, sponsors must maintain a record for each program in a secure location for seven years. These records must indicate the title of the program number, name and a means to identify individuals who earned contact hours, and the date each individual completed the program.

Include a narrative to describe:

  • The Target Population (for whom is the program designed?) Is the program designed for new or experienced nurse educators?
  • Indicate which of The Core Competencies of Nurse Educators identified by the Task Group on Nurse Educator Competencies are addressed in the program.
  • Documentation of the Need for the Program. What evidence is there that the target population needs the material presented in this program? Include information from the literature, surveys, and member requests to support the need for the program.
  • For on-line or self study programs, provide Data regarding the calculation of time it takes learners to complete an online or self study program.
  • Evaluation of Learning. Copies of tests, case studies, and other evaluation tools must be included with the application.
  • Participants’ evaluation of their satisfaction with the speaker(s), content, and program details (environment, AV materials, registration, etc). Copy of the evaluation tool must be included with the application
  • Distribution of Certificates. How will participants receive their certificate or be notified of unsuccessful achievement of the objectives?
COLUMN FORMAT GUIDELINES
The column format must be used and must be typed.
  • State the Learning Objectives clearly in behavioral terms. They should flow from the need for the program and reflect the target population’s level of experience/education.
  • Outline the content in enough detail to convey the essence of the material. Rewording the objective is not adequate.
  • The name of the faculty/speaker presenting the content must be included for each objective.
  • Describe how the learner has opportunity for interaction with the faculty/speaker/expert.
  • Indicate how much time is allotted for each objective or "cluster” of content.
  • Specify the instructional methods for each objective or “cluster” of content.
  • Detail how you will evaluate learning and provide feedback to learners for each learning objective.
  • Specify the method by which overall learning will be evaluated at the end of the learning event.

  1. Program Information and Fees Form
  2. Narrative
  3. Column Format (DOC). Please refer to the guidelines and sample provided to complete the Column Format.
  4. Copy of Program Evaluation Tool
  5. Copy of Learning Evaluation Tool(s)
  6. Biographical form (DOC) for each speaker/faculty/expert contributor
  7. Read the NLN Professional Conduct/Nondiscrimination policy
  8. Read the Policy on Vested Interest. Download and complete the Declaration of Vested Interest form
  9. Check or money order
