NLN Biographical Sketch and Consent Form for
2012 Elections

The NLN Nominations Committee will use the information it receives via this form to consider you for the open positions you have agreed to run for. You will be notified if you have been selected for the ballot.

Special Note:
Before logging in, please gather your information to complete the sections below. This will help expedite the completion of your form. Please remember once there are 20 minutes without activity on the form, your session will time out and your information will not be recorded.

  1. Education and training strengths
  2. Major positions held in the past 10 years
  3. NLN activities in the past 10 years
  4. Relevant governance experience on boards
  5. Pertinent major activities in other organizations during the past 10 years
  6. Experience with national or international policy/decision-making bodies
  7. Membership in professional associations, honor societies, academies, institutes, etc.
  8. You will be asked to identify (100 words each) your unique strengths related to your desired position(s).
  9. In lieu of accepting CVs, your application will be considered based upon your professional, educational, national, regional, and local governance experience. If you wish the nominations committee to take into account other pertinent contributions, then please include in a separate document (MSWord), a list of selected peer-reviewed abstracts, publications, and grants in chronological order.
  10. A reference that the Nominations Committee can contact regarding your candidacy.
