Identification of the Problem

Procedural skills training is essential for preparing advanced practice providers. One key component is learning Incision and Drainage (I&D) for effective abscess management. While numerous commercially available and in-house abscess models exist, many present challenges such as poor anatomical accuracy, high cost, complex or time-intensive assembly, and limited shelf life.

Our simulation program supports over 1,200 Nurse Practitioner (NP), Clinical Nurse Specialist (CNS), and Physician Assistant (PA) learners annually. Because each model is single-use by nature of the procedure, we needed a solution that was easy to produce, cost-effective, and anatomically realistic. Our goal was to design a model that enhances understanding of indications, contraindications, and potential complications, ensuring learners gain the knowledge and confidence to perform I&D competently during clinical rotations and in professional practice.

Unique Idea

Our approach uses low-cost, readily available store-bought items to mass produce shelf-stable anatomical hand models for abscess I&D training. Each model is designed to allow learners to perform the entire procedure from incision to drainage while remaining simple to assemble and store as a preassembled unit for extended periods without refrigeration. This method enables advance production at scale, ensuring models are available on demand without requiring specialized equipment or significant labor.

Objectives

  • Utilizing an individual anatomical abscess model, the advanced practice learner will apply nursing knowledge to evaluate the simulated abscess, practice complex decision-making regarding appropriate treatment, and competently perform the complete Incision and Drainage (I&D) procedure in a safe, independent environment.

Supplies/Ingredients

  1. Kitchen glove
  2. Liquid laundry detergent pod
  3. Cat litter (light weight)
  4. Rubber bands

Steps to Creating the Solution

1. Gather supplies.

2. Open the kitchen glove and add cat litter to cover finger holes.

3. Insert the laundry detergent pod.

4. Fill the remainder of the glove with cat litter until about ¾ full.

5. Secure the glove opening tightly with a rubber band.

6. Verify the pod’s placement by gently feeling the dorsal (back) side of the glove.
