Identification of the Problem

Using IVs on manikins have several challenges. They take effort to apply, use supplies every time they are applied, and aren't always functioning (i.e. fluids cannot be run through them).

Unique Idea

The idea is creating a reusable IV that can quickly be placed on a manikin arm in any position where an actual IV normally would be placed. The reusable IV is functional. This allows students to perform IV pushes and run fluids through a pump.

Objectives

Objective 1 is to allow student to perform IV therapy on the manikin as they would on a real patient. They are able to practice pushing fluids at the correct rate. This also allows them to program IV pumps and start them. As with real life cases, it may force them to work out any air bubbles or occlusions on the IV line. Objective 2 is to reduce the amount of time and supplies needed to perform this setup by making it reusable.

Supplies/Ingredients

  • large nipple cover (choice of color to match manikin)
  • transparent dressing, such as Tegaderm (smaller than the chosen nipple cover)
  • IV catheter (caulking pin on hub and choice of size)
  • extension set
  • spare tubing (at least 3-4 feet long)
  • fluid collection bucket or bed pan
  • butterfly clip
  • optional: blue silicone tape, IV netting, stick glue

Steps to Creating the Solution

  1. Using the chosen size of IV catheter, place the IV through the center of the nipple cover allowing enough space for the transparent dressing to be placed on the nipple cover. The caulking pin on the end of the IV hub needs pushed through to the backside. When placing the IV through, temporarily peel back the plastic layer on the bottom of the nipple cover. Remove the IV needle and dispose in the sharps.
  2. Cut a piece of tubing to fit on the caulking pin of the IV hub. Slide the tubing over the caulking pin on the underside of the nipple cover. It should fit tight creating a seal, so fluids do not leak. Use lubricant if needed.
  3. Attach the extension set. Place the transparent dressing over the hub allowing it to be viewable. Tape down the extension set below the hub. Keep as much of the tape and transparent dressing on the nipple cover to allow more times to reuse.
  4. Clip the other end of the long tubing to a bucket or bedpan with a butterfly clip to be placed out of sight under the bed. Be sure any clamps on this extra tubing are open or removed completely so fluid flow is not restricted.
  5. Remove the plastic backing and keep in a safe place to put back on when storing contraption. Stick the IV in an area that would usually have an IV.
  6. Options: Use extra blue silicone tape to secure the tubing. Extra tubing can be hidden with IV netting. If the nipple cover has issues sticking, there are various methods to help with the adhesion process such as stick glue. Please test these on the manikin before use. If the cover gets dirty, it can also be carefully rinsed with soap and water to help restore the tackiness.
