Identification of the Problem

Novice nursing students entering their maternal-child clinical practicum have lack of exposure to normal breastfeeding newborn stools. Additionally, with early hospital discharge for some moms, some students may not have the opportunity to observe the transitional and breast milk newborn stools in hospital. Likewise, on the postpartum home visit, students may or may not have the opportunity to observe a transitional and breast milk newborn stool.

Unique Idea

Create three newborn stools for use in simulation.

  • I use the meconium stool in a postpartum assessment of mom and newborn simulation that I created. Students receive mom and baby from Labor and Delivery (L and D) and are to perform the initial assessments for both. The report from the L and D registered nurse indicates the newborn voided at birth but has not had first meconium yet. During the first assessment of the newborn, students find meconium in the diaper and are to include this finding during their documentation of assessments on the newborn chart.
  • In a postpartum home visit simulation, I created assessment stations that students rotate through. One station contains the three newborn stools (meconium, transitional, and breast milk) in newborn diapers. Students are asked to identify each stool.

Objectives

Assist students to identify normal newborn stools (meconium, transitional, breast milk).

Supplies/Ingredients

  1. Penaten cream
  2. yellow mustard
  3. sesame seeds
  4. black food coloring

Steps to Creating the Solution

  • Meconium: Combine 15ml of Penaten cream and approximately 150 drops of black food coloring in a medication cup. Keep adding black food coloring to the Penaten cream until the mixture turns black. Mix well and place on diaper. This creates the black color and sticky consistency of meconium. 
  • Transitional Stool: Combine 15ml of yellow mustard and 3 drops of black food coloring in a medication cup. Mix well and place on diaper. This creates the dark green color and looser consistency of transitional stools.
  • Breast Milk Stool: Combine 15ml of yellow mustard and 1 tsp of sesame seeds. Mix well and place on diaper. This creates the yellow color and mushy, watery, seedy consistency of the breast milk stool.
